I use paper and pencil and pen to draw out basic roughs of the pages, then i use MangaStudio EX 4 (Also known in Japan as ComicStudio) to do most of the inking and toning. After that I use photoshop to finish the pictures. The pages tend to look good because I draw at 600dpi (for printing purposes), but when you shrink it down to web size (72dpi), all the flaws are too small to be seen.
